Dear Patients,
Due to the recent rise in covid-19 cases, all patients must follow the steps listed below prior to entry into the clinic. No exceptions will be made.

Fill out a Health Questionnaire and Release of Liability at every single appointment.

Have your forehead temperature taken and recorded by a receptionist.

This is done with a no-contact, infrared thermometer. We will not take temperatures on the wrist.

Swish with a 1% hydrogen peroxide mouth rinse.

Use the hand sanitizer that we provide. You may use your hand sanitizer in addition to ours.

Wear your mask when inside the clinic at all times except when seated in the operatory. You must wear a mask when checking out at the end of your appointment. If you are unable to wear a mask for medical reasons, a face-shield is required. If you do not have a face-shield, we can provide one for a fee.
Only patients are allowed in the clinic. The only “non-patient” individuals allowed to enter are a maximum of 2 caregivers for those with disabilities, a singular parent of a child under 18, and patients requiring a translator. All non-patients must complete the above listed steps prior to entry.
Children are not allowed to attend their parent’s appointments. Patients who come with their children will be rescheduled for a day when they are able to find childcare.
Our patient bathroom is effectively closed. Please use the bathroom near the McDonalds prior to your appointment.
